Is this a validated screening instrument?

No. It is an original reflective check-in, not a validated scale, and its result is not a clinical score. If a structured screener is a better fit, the result points you to the PHQ-9 or GAD-7, which are published screening tools — and even those are screeners, not diagnoses.
